COVID-19 Health & Safety and Risk Acknowledgement
Endeavor Business Media will operate the Event in accordance with official federal and state government and local authority rules, following CDC guidelines and any additional venue or location-specific rules designed to mitigate the spread and contraction of COVID-19. By registering and attending the Event, you agree to follow all enforced behavior requirements and protocols and follow requests of Endeavor and the venue while on the premises and at the Event regarding COVID-19 mitigation.
Payment must be received prior to the conference. If payment is not received by the conference date, the registration fee must be guaranteed on charge card until proof of payment is provided. Make check payable to Endeavor Business Media/ UA Summit 2021
All cancellations must be made in writing to firstname.lastname@example.org. You may cancel without penalty until April 5, 2021, after which a $150 cancellation fee will be charged or 100% of the registration can be transferred to another UAI event. No-shows and cancellations after April 26, 2021 will be charged the full conference rate. All credit card refunds will be issued to the same credit card used during the original registration process.
